Overview of the Role:

As an Occupational Therapist, you will play a crucial role in enhancing the lives of participants by evaluating, reassessing, and providing therapeutic exercises based on referrals from the Interdisciplinary Team. You will design and oversee comprehensive occupational therapy programs, develop functional maintenance plans, and contribute significantly to the rehabilitation and well-being of PACE participants. This position allows you to make a powerful impact in the community, fostering independence and improved quality of life for those you serve.

Responsibilities:

  • Conduct assessments:
    • Gather and compile participant data.
    • Identify overt problems and priorities.
    • Set realistic and measurable goals.
  • Develop care plans:
    • Formulate personalized, multidisciplinary care plans.
    • Collaborate with participants and their families.
  • Implement and manage care:
    • Align therapies with industry and organizational standards.
    • Provide participant education.
    • Manage equipment inventory and home-based interventions.
  • Educate participants and families:
    • Coordinate 24-hour care delivery.
    • Facilitate family and staff training.
    • Conduct home visits as needed.
  • Evaluate care delivery:
    • Observe and document participant care and progress.
    • Make adjustments and solve related problems.
  • Lead and supervise:
    • Guide program aides.
    • Integrate cost-effective measures.
    • Ensure safe and efficient practice environments.
  • Engage in teamwork:
    • Participate in interdisciplinary team meetings.
    • Contribute to care planning processes.
  • Manage additional projects as required.

Requirements:

  • Graduate of an accredited Occupational Therapy program.
  • Valid Occupational Therapy License in good standing.
  • Minimum 2 years of clinical experience with adults with physical disabilities.
  • Advanced practice in Dysphagia preferred.
  • Experience with elderly populations preferred (training provided if needed).
  • Reliable transportation, valid driver’s license, and auto insurance.
  • Medically cleared for communicable diseases and current vaccinations.
  • Successful completion of a comprehensive background check.

Skills and Abilities:

  • Teamwork and collaboration skills.
  • Strong communication with interdisciplinary team members.
  • Clinical competency with geriatric populations.
  • Familiarity with therapy equipment and senior-focused technology.
  • Proper body mechanics for injury prevention and teaching.

Work Environment/Demands:

  • Hands-on care in a rehabilitation gym and Adult Day Health Center.
  • Physical demands, including lifting and assisting participants.
  • Collaboration with clinical and social services disciplines.
