Job summary

The Chateau at Brooklyn Rehabilitation and Nursing Center in New York seeks a skilled Occupational Therapist to join our team. You will assess referred patients, plan and implement rehabilitation programs, and work under physician direction to set realistic goals.

You will supervise OT Assistants, ensure accurate documentation, and collaborate with a multidisciplinary team to deliver patient-centered care for long-term/geriatric patients.

Qualifications

• Must have graduated from an accredited Occupational Therapist program.
• Must hold a valid OT license and state registration.
• Experience supervising Occupational Therapy Assistants.
• Experience in long-term or geriatric care preferred.
• Proficient in documentation writing and clinical records.
• Experience with evaluations, progress notes, re-certification, discharge docs.
• Strong collaboration with multidisciplinary teams.
• Excellent patient service and clinical excellence.
• Punctual and professional, adheres to hours.
• Performs duties as directed by Director of Rehabilitation.
• Confidentiality and HIPAA compliance.

Responsibilities

• Assess referred patients for rehabilitation needs and provide direct patient care.
• Plan and implement rehabilitation programs with goals based on patient capacity.
• Document patient condition and educate patients and families.
• Set realistic goals and monitor progress.
• Supervise Occupational Therapy Assistants and coordinate with the team.

Occupational Therapist

The Chateau is looking for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.

• Must have a valid professional license and registration for state of practice.

• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.

• Experience with long-term care and/or geriatric care is preferred but not limited to.

• Well versed with documentation writing

• Evaluations, weekly progress notes, re-certification and discharge documentations.

• Collaborates with all disciplines to provide best patient centered treatment and care.

• Exceptional patient service skills and ability to demonstrate clinical excellence

• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.

• Perform all other duties assigned by Director of Rehabilitation

• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: FULL TIME

Wage: $40 to 53/hour ( Based on experience)
