Creative Solutions in Healthcare, Inc. is hiring a dedicated Occupational Therapist (OT) to join our in-house team. You will evaluate residents, develop care plans, and support independence through meaningful activities, with emphasis on safety and functional outcomes.

You will collaborate with an interdisciplinary team, modify plans as needed, and document thoroughly in accordance with policies and regulations. Texas licensure and SNF experience are preferred.

Qualifications

• Active Texas OT license in good standing with regulatory boards.
• Experience in SNF or long-term care preferred.
• Familiarity with Net Health EDS.
• Ability to adapt to diverse patient needs.
• Strong communication and collaboration skills.

Responsibilities

• Evaluate patients and assess impact on daily activities.
• Develop and implement individualized treatment plans.
• Recommend home modifications and adaptive equipment.
• Document thoroughly and timely per policies.
• Collaborate with interdisciplinary team.
• Provide education to patients and families.
• Monitor response to treatment and adjust as needed.
• Attend required meetings with Rehab Director.

Join Our Team as a Full-Time Occupational Therapist (OT)

We are seeking a dedicated and compassionate Occupational Therapist (OT) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care focused on improving residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes.

Your Impact as an Occupational Therapist (OT)

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Assess patients’ home and living environments and recommend modifications based on their needs and capabilities
• Recommend adaptive equipment and provide training to support independence and safety
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical education and support to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Occupational Therapy license
• In good standing with all regulatory agencies and licensing boards
• Skilled Nursing Facility (SNF) or long-term care experience preferred
• Current knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision‑making skills
