Job summary

Jackson Therapy Partners is hiring a licensed Occupational Therapist for a flexible home health role serving patients in the Creston and Winterset area of IA. You will help patients improve function, safety, and independence in their homes.

This position welcomes new graduates and supports licensed professionals with a focus on individualized, patient-centered care while traveling within the Creston/Winterset service areas. Eligible for full-time benefits.

Qualifications

• OTA must have an OT degree from an accredited program.
• Active Iowa OT license required or ability to obtain before start.
• BLS may be required from AHA or ARC.
• Home health experience preferred; hospital or SNF is a plus.
• Ability to travel within Creston and Winterset service areas; new grads welcome.

Responsibilities

• Provide OT services in a home health setting to improve function, safety, and independence.
• Assess patient needs and develop individualized treatment plans.
• Collaborate with patients and families to support progress and goals within the home environment.
