Job summary

Tier1usa in Illinois seeks an licensed Occupational Therapist (OT) for a full-time role at a Skilled Nursing Facility/Long-Term Care setting. The compensation is $36.00–$43.00 per hour and requires active state licensure.

You will evaluate, treat, and develop personalized care plans to improve functional outcomes for our patients. The renewal rehab team emphasizes collaboration, ongoing clinical education, and a supportive work environment with flexible scheduling and opportunities for growth

Qualifications

• Degree in Occupational Therapy from an AOTA-accredited program.
• NBCOT certification as an OTR (if required by state).
• Current OT license in the practicing state.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
