Job summary

Renewal Rehab is hiring a Licensed Occupational Therapist (OT) for a full-time position in a Skilled Nursing Facility/Long-Term Care setting in Illinois. The role requires current OT licensure and NBCOT certification as applicable by state.

You will evaluate and treat patients, develop personalized care plans, and collaborate with the team to ensure quality outcomes, while maintaining proper documentation and compliance with orders.

Qualifications

• OT degree from an AOTA-accredited program.
• NBCOT certification as an OT-registered (if required by state).
• Current OT license in the state of practice.
• Strong communication and organizational skills.
• New grads welcome; mentorship available.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments
• Create and implement personalized care plans
• Collaborate with team to ensure quality outcomes
• Maintain documentation to meet regulatory requirements
• Ensure all required physician orders are obtained and up to date
