Responsibilities

• Effectively screen and evaluate patients with cognitive and/or ADL disorders.
• Utilize standardized assessments to support clinical interventions.
• Provide skilled, medically necessary treatment interventions.
• Administer therapeutic activities, exercises, and modality interventions.
• Assess patient progress and modify treatment plans as appropriate.
• Evaluate treatment outcomes and make necessary referrals.
• Educate and counsel patients and healthcare providers.
• Communicate effectively with supervisors and team members.
• Complete clinical documentation and discharge summaries promptly.
