Renewal Rehab in Charleston, IL is seeking a licensed Occupational Therapist for a full-time role in a skilled nursing facility and long-term care setting. You will evaluate and treat patients, create care plans, and collaborate with the care team to achieve quality outcomes.

Qualifications

• Must have a degree in Occupational Therapy from an AOTA-accredited program.
• NBCOT certification as an OTR may be required by the state.
• Current license as an Occupational Therapist in the state of practice.
• Strong communication and organizational skills.

Responsabilités

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all physician orders are obtained and up to date.
