• Must have an accredited Occupational Therapy degree recognized by AOTA.
• SNF experience preferred for this role.

Responsibilities

• Perform comprehensive OT evaluations based on MD orders.
• Develop and document treatment plans with goals, frequency and duration.
• Provide therapy to residents using approved modalities and adjust as needed.
• Ensure proper documentation including progress notes, recertifications and discharges.
