Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care; develop compliant care plans.
• Deliver rehabilitative and medically necessary treatments.
• Perform regular standing/walking and assist with transfers; manage patient care throughout shifts.
• Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises and wound care.
• Provide counseling to patients, families, and other providers about physical disorders.
• Generate discharge summaries ensuring regulatory compliance.
