Qualifications

• Active Illinois OT license required.
• Experience in SNF/long-term care preferred.
• Proficient in current treatment practices.
• Familiarity with PCC Electronic Documentation System preferred.

Responsibilities

• Develop personalized treatment plans to maximize safety and performance in ADL/IADL.
• Ensure all treatment aligns with the established plan of care.
• Monitor patient response and adjust treatment with IDT input.
• Maintain thorough documentation compliant with regulations.
