Job summary

Warren Center is seeking a compassionate and skilled Occupational Therapist to join our interdisciplinary rehabilitation team. You will design and implement personalized treatment plans to promote independence, safety, and quality of life for residents.

You will collaborate with the rehab team, educate residents and families on therapeutic exercises and techniques, monitor progress, and maintain accurate patient records. Licensed Occupational Therapist required.

Qualifications

• Licensed Occupational Therapist credential required.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with interdisciplinary team to ensure coordination of care
• Educate residents and their families on therapeutic exercises and techniques
• Monitor and report on resident progress and outcomes
• Maintain accurate and up-to-date records of patient care
