Job summary

Liberty Healthcare and Rehabilitation Services is actively seeking an experienced Occupational Therapist to join our multidisciplinary rehab team in Charlotte. You will evaluate patients, develop individualized care plans, and implement restorative programs while coordinating with physicians and staff.

This role emphasizes accurate documentation, productivity, and collaboration with the rehab team. NC licensure and NBCOT certification for new graduates are required, with prior SNF or long-term

Qualifications

• Must be a graduate of an accredited Occupational Therapy program.
• Must meet NC Board criteria; NBCOT certification for new grads.
• SNF or long-term care experience preferred.
• Strong documentation and time-management skills.

Responsibilities

• Respond to requests for service and obtain physician orders prior to patient evaluations.
• Evaluate patients and establish individualized plans of care featuring objective, measurable, and functional goals.
• Assess treatment effectiveness regularly and modify plans of care as clinically indicated.
• Establish restorative and/or maintenance programs tailored to individual patient needs.
• Plan, administer, and supervise therapy programs, including the supervision and training of COTA’s and OT aides.
• Communicate effectively with referring physicians, interdisciplinary rehab team members, and facility staff regarding total treatment programs.
• Maintain accurate, timely, and complete daily treatment records and weekly progress notes in accordance with documentation guidelines.
• Travel to other locations to provide therapy services as needed.
