Job summary

Choice Health At Home is seeking a Certified Occupational Therapy Assistant (COTA) to join our home health therapy team in Broomfield, CO. This full-time role focuses on one-on-one patient care in the home setting, aiming to improve independence and daily function after illness or surgery.

Requirements include an active COTA license, CPR, and a valid driver’s license with reliable transportation. Home health experience and HCHB familiarity are preferred to support seamless care delivery.

Qualifications

• Must hold an active Certified Occupational Therapy Assistant (COTA) license.
• CPR certification current.
• Driver's license with reliable transportation.
• Home health or SNF experience preferred.
• HomeCare HomeBase (HCHB) experience preferred but not required.
• Ability to perform hands-on patient care in home environments.
• Lift up to 100 lbs as needed.

Responsibilities

• Provide occupational therapy treatments in home health settings under the direction of an Occupational Therapist.
• Assist patients with activities of daily living (ADLs) such as dressing, bathing, grooming, and mobility.
• Help improve strength, coordination, balance, and functional independence.
• Deliver care focused on rehabilitation, recovery, and quality of life.
• Follow infection control, safety, and universal precautions.
• Maintain accurate and timely clinical documentation.
• Communicate with patients, families, and care teams.
• One-on-one care in home environments.

Salary Range: $65,000.00 To $95,000.00 Annually

We are seeking a Certified Occupational Therapy Assistant (COTA) to join our growing home health therapy team. What You’ll Do

• Provide occupational therapy treatments in home health settings under the direction of an Occupational Therapist
• Support patients with activities of daily living (ADLs) such as dressing, bathing, grooming, and mobility
• Assist in improving strength, coordination, balance, and functional independence
• Deliver care focused on rehabilitation, recovery, and quality of life improvement
• Follow infection control, safety, and universal precautions
• Maintain accurate and timely clinical documentation
• Communicate effectively with patients, families, and interdisciplinary care teams
• Active Certified Occupational Therapy Assistant (COTA) license
• Current CPR certification
• Valid driver’s license and reliable transportation
• Home health or skilled nursing facility experience preferred
• Experience with HomeCare HomeBase (HCHB) preferred but not required
• Ability to perform hands-on patient care in home environments
• Must be able to stand, walk, squat, kneel, crawl, and climb as needed
• Ability to lift, carry, push, or pull up to 100 lbs
