Optional COTA supervision is available. Competitive pay per visit with W-2 and 1099 options and a collaborative clinical team are offered.

Qualifications

• Current Arizona Occupational Therapy license is required.
• CPR certification is required.
• AHCCCS ID is required.
• Article 9 certification is required.
• Valid driver's license and dependable transportation.
• Excellent clinical judgment is essential.
