Job summary

Community Physical Therapy in Woodridge is seeking a PRN Occupational and Physical Therapist to join our Admit-to-Discharge model. You will manage a caseload that fits your availability and follow patients from initial evaluation through discharge. This role offers $55 per hour with flexible weekday scheduling between 7:00 a.m.

and 6:00 p.m. and opportunities in outpatient and home-care services within an Independent Living community, enabling you to build lasting relationships and deliver

Qualifications

• Licensed PT/OT required by state.
• PRN role with admit-to-discharge caseload.
• Excellent communication and bedside manner.

Responsibilities

• Manage caseload from initial evaluation through discharge.
• Provide outpatient and home care within an Independent Living community.

Community Physical Therapy (CPT) is seeking a PRN Occupational and Physical Therapist to join our unique Admit-to-Discharge model.

Unlike a traditional PRN role, you'll manage a caseload that fits your availability and follow your patients from their initial evaluation through discharge. This continuity of care allows you to see your patients' progress, build lasting relationships, and make a meaningful impact throughout their recovery.
