Silas Creek Rehabilitation Center in Winston-Salem, North Carolina, is seeking a dedicated Occupational Therapist (OT) to join our growing rehabilitation team. This full-time opportunity focuses on improving Residents’ quality of life and independence through individualized therapy programs in a skilled nursing setting.

You will work collaboratively with an interdisciplinary team to provide exceptional care in a supportive, team-oriented environment.

Formación

• Bachelor’s or Master’s Degree in Occupational Therapy (OT).
• Current North Carolina Occupational Therapy license required.
• NCBOT certification/registration required.
• Skilled nursing or long-term care experience preferred.
• Strong communication, organization, and teamwork skills.

Responsabilidades

• Evaluate Residents and develop individualized treatment plans.
• Implement occupational therapy services according to physician orders and care plans.
• Monitor Resident progress and adjust treatment plans as needed.
• Maintain accurate and timely documentation.
• Collaborate with nursing, therapy, and interdisciplinary team members.
• Educate Residents and families regarding therapy goals and progress.
• Ensure compliance with federal, state, and facility guidelines.

Schedule

• Flexible scheduling available

• Weekend rotation as needed

Occupational Therapist Responsibilities
