Qualifications

• Active and unrestricted California Occupational Therapist (OT) License.
• Strong clinical assessment and treatment skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled, evidence-based occupational therapy interventions.
• Help residents improve daily living skills, safety, mobility, and functional independence.
• Collaborate with Physical Therapists, Speech-Language Pathologists, nursing staff, physicians, and interdisciplinary care teams.
• Educate residents, families, and caregivers regarding treatment goals and progress.
• Maintain accurate, timely, and compliant documentation.
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.

• Experience in Skilled Nursing Facilities (SNF), Post-Acute Care, Long-Term Care, Rehabilitation, or Geriatric Therapy
• Ability to work independently and as part of a multidisciplinary team
