Job summary

Personal Healthcare Management LLC seeks a Licensed Occupational Therapist to assess and treat residents in a skilled nursing facility in New York. You will develop Plan of Care, perform evaluations per physician order, and supervise staff as needed.

The role requires NYS OT licensure, a degree in OT, and ability to work independently or with a team; geriatric experience is preferred. Knowledge of Medicare/Medicaid and regulatory standards is essential.

Qualifications

  • Must have a current unencumbered NYS license as an OT.
  • Graduate of an accredited OT program; international equivalents considered.
  • Geriatric experience preferred and valid driver’s license.

Responsibilities

  • Screens residents for rehabilitation therapy needs.
  • Completes OT evaluations upon physician order.
  • Establishes OT Plan of Care for treatments per physician order.
  • Provides OT plan of treatments and modalities.
  • Modifies treatment plan based on resident response.
  • Documents progress toward OT goals.
  • Discharges residents from OT services when appropriate.
  • Recommends mobility, positioning, ROM and self-care/ADL plans to nursing.
  • Completes daily schedule, progress notes and re-certifications.
  • Supervises OT Assistants and unlicensed personnel.
  • Educates residents, staff and families; participates in in-servicing.
  • Maintains Medicare Part A PPS case management knowledge.
  • Complies with federal/state rehab regulations and documentation standards.
  • Works toward productivity goals and initiates corrective action when needed.
  • Completes care plans and MDS as directed; orders equipment as needed.
  • Participates in discharge planning with interdisciplinary team.

Qualifications/Experience:
  • Must have a current unencumbered NYS license as a Registered Professional Occupational Therapist, Bachelor's degree or higher.

  • Graduate of an accredited school of occupational therapy or international equivalent

  • A current unrestricted driver's license and dependable transportation

Essential Job Functions:
  • Screens residents for rehabilitation therapy needs

  • Completes Occupational Therapy evaluations upon physician order

  • Establishes Occupational Therapy Plan of Care for treatments per physician order

  • Provides Occupational Therapy plan of treatments and modalities

  • Modifies treatment program level, intensity and techniques based on the resident's response to specific treatment plan

  • Documents resident progress toward occupational therapy goals

  • Discharges residents from Occupational Therapy services when appropriate

  • Makes appropriate recommendations for nursing regarding mobility, positioning, ROM and Self-care/ADL tasks

  • Completes Daily Activity Schedule, progress notes and re-certifications per facility policy.

  • Supervises Certified Occupational Therapy Assistants and unlicensed assistive personnel

  • Provides resident, staff and family education and in-servicing

  • Demonstrates working knowledge of Medicare Part A PPS case management

  • Complies with Federal and State regulations related to rehabilitation therapy

  • Completes all documentation to comply with professional standard and regulatory requirements

  • Strives to meet productivity / efficiency goals and self-initiates corrective action plan for non-attainment

  • Completes care plans and MDS as directed, per facility policy

  • Assists in ordering necessary equipment for the facility and residents

  • Participates in the discharge planning process with the interdisciplinary team

  • Participates in facility meetings and committees as assigned

  • Submits required reports and documentation in accordance with established acceptable time frames

  • Possess a strong knowledge of Medicare, Medicaid and other payment sources and their responsibility in the SNF setting

  • Maintains current professional knowledge of practice of Occupational Therapy and practices within acceptable professional standards

  • Complies with all facility policies and procedures

  • Maintains confidentiality of facility and resident information

  • Respects all residents all resident rights

  • Reports all instances of potential resident abuse, neglect, and misappropriation of resident property immediately

  • Follows sanitation and infection control policies and procedures

  • Keeps Facility Rehabilitation manager informed and updated appropriately

  • Recommends corrective action plan for all identified problems

  • Maintains a positive working relationship between the rehabilitation department and other departments
