Job summary

Highland-Rehabilitation-and-Nursing-Center is hiring for Occupational Therapist in a skilled nursing facility setting in Middletown, NY. The OT screens residents, evaluates needs, and develops therapeutic plans of care with physician orders, delivering treatments and adapting to resident responses.

The role requires NYS OT licensure, a Bachelor’s degree in OT, and preferably geriatric experience. Strong communication, teamwork, and ability to work in a regulated environment are essential.

Qualifications

  • Must have NYS license as Registered Occupational Therapist.
  • Bachelor’s degree or higher in OT required.
  • Graduate of an accredited OT program or international equivalent.
  • Geriatric experience preferred.
  • Valid NY driver’s license and dependable transportation.
  • Able to communicate professionally with internal and external resources.
  • Able to work independently and as part of a team.
  • Able to function well in stressful and changing situations.
  • Able to understand and follow oral and written directions.

Responsibilities

  • Screens residents for rehabilitation therapy needs.
  • Completes Occupational Therapy evaluations upon physician order.
  • Establishes OT Plan of Care per physician order.
  • Provides OT plan of treatments and modalities.
  • Modifies treatment based on resident response.
  • Documents resident progress toward OT goals.
  • Discharges residents from OT services when appropriate.
  • Makes recommendations for mobility, positioning, ROM and ADL tasks.
  • Completes Daily Activity Schedule, progress notes and re-certifications.
  • Supervises Certified OT Assistants and unlicensed staff.
  • Provides education to residents, staff and families.
  • Maintains knowledge of Medicare Part A PPS case management.
  • Complies with federal/state rehab regulations.
  • Completes documentation per professional standards and policy.
  • Meets productivity/efficiency goals and takes corrective action as needed.
  • Completes care plans and MDS as directed.
  • Assists in ordering equipment for facility and residents.
  • Participates in discharge planning with interdisciplinary team.
  • Attends facility meetings and committees as assigned.
  • Submits reports within required timeframes.
  • Knows Medicare, Medicaid and payment sources in SNF settings.
  • Keeps current OT practice knowledge and standards.
  • Follows facility policies and protects confidentiality.
  • Respects resident rights and reports abuse or neglect promptly.
  • Follows sanitation and infection control policies.
  • Keeps Rehab manager informed of updates.
  • Suggests corrective actions for identified problems.
  • Maintains positive relationships between rehab and other departments.

Education

Bachelor’s degree in Occupational Therapy
Accredited OT program graduate

  • Discharges residents from Occupational Therapy services when appropriate

  • Makes appropriate recommendations for nursing regarding mobility, positioning, ROM and Self-care/ADL tasks

  • Completes Daily Activity Schedule, progress notes and re-certifications per facility policy.

  • Supervises Certified Occupational Therapy Assistants and unlicensed assistive personnel

  • Provides resident, staff and family education and in-servicing

  • Demonstrates working knowledge of Medicare Part A PPS case management

  • Complies with Federal and State regulations related to rehabilitation therapy

  • Completes all documentation to comply with professional standard and regulatory requirements

  • Strives to meet productivity / efficiency goals and self-initiates corrective action plan for non-attainment

  • Completes care plans and MDS as directed, per facility policy

  • Assists in ordering necessary equipment for the facility and residents

  • Participates in the discharge planning process with the interdisciplinary team

  • Participates in facility meetings and committees as assigned

  • Submits required reports and documentation in accordance with established acceptable time frames

  • Possess a strong knowledge of Medicare, Medicaid and other payment sources and their responsibility in the SNF setting

  • Maintains current professional knowledge of practice of Occupational Therapy and practices within acceptable professional standards

  • Complies with all facility policies and procedures

  • Maintains confidentiality of facility and resident information

  • Respects all residents all resident rights

  • Reports all instances of potential resident abuse, neglect, and misappropriation of resident property immediately

  • Follows sanitation and infection control policies and procedures

  • Keeps Facility Rehabilitation manager informed and updated appropriately

  • Recommends corrective action plan for all identified problems

  • Maintains a positive working relationship between the rehabilitation department and other departments
