Job summary

Responsibilities

• Assures efficient management of work processes.
• Provides initial evaluation of patient's abilities and designs appropriate intervention plans.
• Treats patients with therapeutic procedures to improve function.
• Consults with patients to learn about their physical condition and referral details.
• Reviews medical history and referrals from doctors or surgeons.
• Diagnose movement dysfunction by observation and patient movement.
• Develops individualized treatment plans.
• Maintains patient records and tracks goals.
• Uses standardized assessments and evidence-based practice.
• Assesses progress and adjusts treatments as required.
• Evaluates outcomes and refers appropriately.
• Provides counseling to patients and families about physical disorders.
• Communicates progress and plans with supervisor and health team.
• Provides discharge summaries per regulatory requirements.
• Documents information in electronic records and records treatment minutes.
• Participates in trainings and conferences.
• Able to supervise and support assistants and students.
• Adheres to company policies and code of conduct.

Responsibilities

• Assures efficient management of work processes.

• Treats patients with therapeutic procedures, modalities and or equipment to improve function.
• This position reports to the Director of Rehabilitation Services.
• · Consult with patients to learn about their physical condition and symptoms. Review their medical history and referrals from doctors or surgeons.
• · Diagnose movement dysfunction by listening to patients and observing them as they move.
• · Develop individualized treatment plan
• · Maintain patient record by keeping track of goals
• · Utilize standardized assessments and evidence-based practice to support clinical interventions.
• · Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• · Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• · Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• · Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• · Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• · Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• · Participate in facility and company required trainings, in-services and conferences.
• · Able to supervise and support occupational therapist assistants; rehabilitation aides and/or students.
• · Adherence to Company required policy and procedures and code of conduct.
