JTC Home Health Inc. in San Diego is seeking a Part-Time Occupational Therapist to join our team serving patients in the San Diego area and North County.

If you excel at helping patients regain independence in their daily lives, we want you! You will conduct evaluations, develop and manage personalized OT plans, and help patients regain independence in ADLs within their homes, collaborating with RNs and other therapists.

Qualifications

• Current, active California OT license.
• Valid Driver’s License and reliable transportation.
• Experience in home health or outpatient rehabilitation is preferred.
• Current CPR/BLS Certification.

Responsibilities

• Conduct comprehensive evaluations of patient function, cognitive skills, and ADL/IADL performance.
• Develop, implement, and manage individualized, goal-oriented OT treatment plans.
• Focus on activities of daily living to maximize independence and safety at home.
• Collaborate with RNs, PTs, and Speech Therapists to ensure coordinated, high-quality care.
• Maintain professional and timely documentation as required by Medicare and state guidelines.

What You’ll Be Doing

• Conducting comprehensive evaluations to assess patient function, cognitive skills, and ADL/IADL performance.

Essential Qualifications

• Current, active California Occupational Therapist (OT) License.
• Valid Driver’s License and reliable, insured personal transportation for community visits.
• Experience in home health or outpatient rehabilitation is highly preferred.
• Strong skills in functional assessment, patient motivation, and goal setting.
• Current CPR/BLS Certification.
