IU Health Fort Wayne is seeking an experienced Occupational Therapist to provide inpatient and acute care services in the Fort Wayne hospital. You will evaluate patients across the lifespan, develop individualized plans of care, and work with a multidisciplinary team to support meaningful daily activities and safe transitions of care.
This role requires a master’s degree in occupational therapy, Indiana licensure, and BLS; NBCOT certification is preferred.

Full Time M-F with rotating weekend coverage; Positions will support inpatient only - intensive care units, progressive care units, and medical-surgical patients.
Provides skilled occupational therapy services to patients across the lifespan whose illness, injury, or condition impacts participation in daily activities and functional independence. Evaluates occupational performance and develops individualized plans of care that address physical, cognitive, psychosocial, sensory, and environmental factors affecting function. Implements evidence-based interventions to improve participation in daily activities, functional mobility, self-care, and other meaningful occupations. Collaborates with patients, families, and the interdisciplinary care team, and supports safe transitions of care to optimize outcomes and quality of life. Supervises occupational therapy assistants, students, and support personnel in accordance with state regulations and organizational policy. Participates in activities that support patient care, quality improvement, education, safety, and department operations while advancing IU Health's mission, vision, and values.

Clinical Practice - Performs occupational therapy evaluations, assessments, treatment planning, treatment interventions, reassessments, and re-evaluations in accordance with the Occupational Therapy Practice Framework. Provides direct patient care to address functional limitations affecting participation in daily activities and occupational performance. Develops individualized plans of care with measurable goals, collaborates with patients, caregivers, and the healthcare team, and identifies discharge planning needs. Completes accurate and timely documentation, coding, and charge capture in compliance with department, regulatory, and payer requirements. Initiates referrals when patient needs fall outside the scope of occupational therapy practice.
