Job summary

Connect Pediatrics is seeking a Pediatric Occupational Therapist to join our team in the Prosper, Texas area. You will provide in-home therapy, collaborate with a multidisciplinary team, and help families navigate care to support medically complex children.

• Develop and implement treatment plans aligned with physician orders.
• Provide in-home therapy services for pediatric patients with a range of diagnoses.
• Engage families with regular communication and updates on goals.
• Collaborate with a multidisciplinary team to address patient needs.
• Maintain comprehensive documentation of care and treatment changes.
• Participate in one-on-one meetings with team leads to discuss progress.

Responsibilities:

• Perform comprehensive evaluations, using standardized tests to determine pediatric patients' needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician's orders.
• Provide in-home therapy services for pediatric patients with a ride range of diagnoses.
• Engage with patients' families, providing them with regular communication and updates on goals for their child.
• Collaborate with a multidisciplinary team, including other therapy disciplines, physicians, and nurses, to develop holistic treatment plans that address the diverse needs of each patient
• Maintain comprehensive documentation that reflects the care provided and communicates changes in treatment plans to the entire healthcare team.
• Actively participate in one-on-one meetings with team leads to discuss patient progress, share insights, and refine treatment strategies collaboratively.
