Job summary

Millennium Physician Group seeks Occupational Therapists to join our Home Health Team serving Charlotte and Sarasota Counties in Florida. You will evaluate patients’ physical, cognitive, and functional abilities, develop individualized treatment plans, and deliver therapies in patients’ homes to promote independence and quality of life.

This Per Diem (PRN) role offers flexible hours with staffing needs, patient volume, and organizational requirements affecting schedules.

Qualifications

• Current and valid Occupational Therapist license in Florida.
• Minimum of 6 months experience in a hospital, skilled nursing facility, or rehab.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct comprehensive assessments of patients’ physical, cognitive, and functional abilities to develop individualized treatment plans.
• Provide hands-on therapy to improve daily living skills, mobility, and independence.
• Educate patients and families on strategies for managing and adapting to conditions.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.

We are seeking Occupational Therapists (OT) to join our Home Health Team in Charlotte and Sarasota Counties. In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high‑performing care team, you will evaluate patients’ physical, cognitive, and functional abilities, develop individualized treatment plans, and provide therapeutic interventions that promote independence and improve quality of life. You will educate patients and families, collaborate with an interdisciplinary care team, maintain accurate documentation, and deliver high-quality, patient-centered care.

This is a Per Diem (PRN) position with variable time and hours can vary from week to week based on staffing needs, patient volume, call-offs, vacations, or organizational requirements.

What You’ll Do

• Patient Care: Conduct comprehensive assessments of patients’ physical, cognitive, and functional abilities to develop individualized treatment plans.


• Therapeutic Interventions: Provide hands‑on therapy to improve patients’ daily living skills, mobility, and independence. Implement therapeutic interventions, including therapeutic exercises, adaptive equipment training, and environmental modifications.


• Patient Education: Educate patients and families on strategies for managing and adapting to their conditions, maximizing their independence and quality of life.


• Collaboration: Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care and ensure optimal outcomes.


• Documentation: Document patient progress, treatment plans, and outcomes accurately and timely.


• Professional Development: Stay up to date with the latest research and advancements in occupational therapy practice.

What We’re Looking For

• Licensure: Current and valid Occupational Therapist license in the state of Florida.


• Experience: Minimum of 6 months experience in a hospital, skilled nursing facility, or rehab.
