Job summary

Immediate Home Health and Hospice is seeking an experienced Occupational Therapist to provide in-home evaluation and therapy to patients in the Philadelphia area. You will work with a multidisciplinary team to deliver coordinated home-based care and ensure treatment plans are executed with high-quality documentation.

The role requires an active OT license, a valid driver’s license, and at least two years of OT experience.

Qualifications

• Active Occupational Therapist state license is required.
• Valid drivers’ license is required.
• Two or more years of occupational therapist experience is preferred.

Responsibilities

• Collaborate with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive care in patients' homes.
• Evaluate patients and develop occupational therapy treatment plans.
• Deliver treatment per care plan and adjust as needed.
• Maintain accurate and timely clinical and progress notes.

Responsibilities

What You Will Do:

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Conduct evaluations and develop occupational therapy treatment plan for our patients
• Deliver treatment/care in accordance with care plan
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Two or more years of applicable occupational therapist experience
