Job summary

Norwood HealthCare is seeking a compassionate Occupational Therapist to join our rehabilitation team in a skilled nursing setting. You will provide cognitively focused and ADL-based therapy to help patients regain independence and improve quality of life.

Responsibilities include evaluating patients, developing plans of care, using standardized assessments, and documenting outcomes while collaborating with families and care teams.

Qualifications

• Bachelor's degree or higher in Occupational Therapy or related field.
• 1+ years of experience in a skilled nursing facility environment.

• Administers procedures as part of the rehabilitation plan including ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices.
• Assesses patient progress and adjusts treatments in the clinical record per regulations.
• Evaluates treatment outcomes and adjusts services based on outcomes.
• Provides consultation and counseling to patients, families, caregivers and other providers.

Pay Range: $40.00 - $55.00

JOB SUMMARY:

The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

JOB DUTIES:

• Effectively screens/evaluates patients and/ with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the patients’ physical disorders

REQUIREMENTS / EDUCATION / EXPERIENCE

• Bachelor's degree or higher in Occupational Therapy or related field
• 1+ years of experience in a skilled nursing facility environment
