Job summary

Livingston Hills Nursing and Rehab in Hudson, NY is actively seeking an Occupational Therapist (OTR) to join our skilled nursing facility team.

You will evaluate residents, develop treatment plans, select meaningful activities, monitor progress, and document care while maintaining confidentiality and a safe environment.

Experience in long-term care is preferred and NY license is required; we offer a collaborative team and opportunities to supervise COTAs.

Qualifications

• Graduate of an accredited Occupational Therapy Program.
• Current New York State OT License.
• Experience in long-term care settings preferred.

Responsibilities

• Evaluate Residents and develop a treatment plan that meets their needs.
• Choose activities that help Residents carry out daily tasks independently.
• Monitor Residents as they carry out the tasks given to them.
• Observe and assess Residents' progress and prepare detailed reports accordingly.
• Help in rehabilitating Residents disabled as a result of injury, illness, or other problems.
• Maintain a safe and clean working environment.
• Maintain Resident confidentiality by monitoring information processing.
• Maintain documentation of Resident care services by auditing records.
• Maintain a cooperative relationship among healthcare teams and contribute to the team effort.
• Supervise COTAs as required.

Education

OT Duties Include:

• Evaluate Residents & develop a treatment plan that will meet their needs.

• Monitor Residents as they carry out the task given to them.
• Observe & assess Residents' progressand prepare detailed reports accordingly.
