Job summary

Best Home Health Providers, Inc. is seeking an Occupational Therapist to help patients regain independence in their homes across California. The role is per diem with visits valued at $110–$140 each.

You will assess patient needs, develop personalized plans, and collaborate with physicians and a care team to ensure safe, high-quality home health care. Ideal candidates hold California OT licensure, CPR, and at least 2 years of clinical OT experience, plus a valid driver's license and reliable

Qualifications

• Graduate of an occupational therapy curriculum accredited by ACOTE.
• Registered with the National Registration Examination of the AOTA.
• Active California OT license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license with reliable insured vehicle and ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in activities of daily living.
• Instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and report changes in patient condition.
• Coordinate with the broader care team for seamless, patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.

• Educate family members and caregivers on the patient's treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

Requirements

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
