Job summary

Lakes Region VNA and Hospice is seeking a dedicated Occupational Therapist to join our traveling team serving Barnstead, Laconia, and surrounding areas. You will provide high-quality therapy services to help patients regain independence and improve their quality of life.

Responsibilities include evaluating needs, developing plans, teaching skills, fabricating splints, and coordinating with physicians and interdisciplinary teams. NH licensure and CPR are required.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Licensed with the NH State Board.
• Minimum of 2 years of clinical experience as an Occupational Therapist.
• Current CPR certification.
• Valid driver's license and reliable, insured vehicle.

Responsibilities

• Evaluate patients’ functional status and home environment for hazards or barriers to independence.
• Develop individualized treatment programs and establish goals for improved function.
• Teach new skills or retrain patients in daily activities lost due to illness or injury.
• Fabricate splints and instruct patients in the use of adaptive equipment, prosthetics, and orthotics.
• Maintain accurate clinical records and communicate changes in patient condition to physicians.
• Participate in care planning, discharge planning, and interdisciplinary meetings.
• Supervise Certified Occupational Therapy Assistants and home health aides as required.
• Ensure compliance with organizational policies and state regulations.
