Toluca Lake Transitional Care in North Hollywood, CA, is seeking a PRN Occupational Therapist to join our in-house rehab program. The role offers flexible scheduling with a competitive hourly pay between $56 and $65 based on experience.

Licensed OT with CA licensure will screen and treat geriatric patients, develop plans of care, and collaborate with the health team. Join our stable, mission-driven post-acute care facility and help patients recover and regain independence.

Qualifications

• Licensed OT with CA licensure.
• Experience in geriatric rehab preferred.
• Willingness to work PRN schedule.

Responsibilities

• Screen/evaluate patients with cognitive/ADL disorders and develop care plans.
• Use standardized assessments and evidence-based practice.
• Provide rehabilitative treatment per regulatory requirements.
• Administer ADL retraining, therapeutic activities, exercises, modalities, and assistive devices.
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and refer as needed.
• Provide counseling to patients and families about physical disorders.
• Communicate progress and plan with the health team.
• Complete discharge summaries per clinical requirements.

Toluca Lake Transitional Care in North Hollywood, CA, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a PRN Occupational Therapist for our in-house rehab program!

PRN : Flexible schedule with work-life balance
Pay : $56 - $65 /hour based on experience

Key Responsibilities

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
