Job summary

AllStar Therapies seeks a compassionate Home Health Occupational Therapist (OT) to provide in-home therapy services. The role focuses on helping patients regain independence and safely remain in their homes with one-on-one, patient-centered care.

The OT will evaluate patients in their homes, develop treatment plans, and collaborate with physicians and the care team. A valid OT license and a flexible schedule are required; home health experience is preferred.

Qualifications

• Current OT license in the state of practice.
• Graduate of an accredited OT program.
• Home health experience preferred, but not required.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently with a flexible daily schedule.
• Excellent communication, documentation, and organizational skills.
• CPR certification preferred.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans.
• Provide skilled occupational therapy services per physician orders and plan of care.
• Assess and improve patients' ability to perform ADLs and IADLs.
• Recommend adaptive equipment, home modifications, and safety strategies.
• Educate patients, families, and caregivers on treatment plans and home exercise programs.
• Complete timely and accurate documentation per guidelines.
• Communicate with physicians, nurses, case managers, and the care team.
• Participate in care conferences and discharge planning as needed.

This rewarding position offers the opportunity to help patients regain independence and safely remain in their homes while delivering one-on-one, patient-centered care.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans
• Provide skilled occupational therapy services in accordance with physician orders and the established plan of care
• Assess and improve patients' ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Recommend adaptive equipment, home modifications, and safety strategies
• Educate patients, families, and caregivers on treatment plans and home exercise programs
• Complete timely and accurate documentation in accordance with agency, state, federal, and Medicare guidelines
• Communicate effectively with physicians, nurses, case managers, and the interdisciplinary care team
• Participate in care conferences and discharge planning as needed
