Qualifications

Responsibilities

• Evaluate patients and create personalized care plans focusing on functional independence and safety.
• Deliver therapy through activities, ADL training and adaptive techniques.
• Assist patients with daily tasks such as dressing, bathing, grooming and meals.
• Collaborate with interdisciplinary teams and caregivers to support outcomes.
• Track progress and maintain clinical documentation; adjust plans as needed.

Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life
• Deliver care through therapeutic activities, ADL training, and adaptive techniques
• Help patients improve their ability to perform daily tasks such as dressing, bathing, grooming, and meal preparation
• Partner with interdisciplinary teams, caregivers, and nursing staff to support strong patient outcomes and continuity of care
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals
