Misty Willow Healthcare and Rehabilitation in North Houston, TX is seeking a PRN Occupational Therapist to join our in-house therapy team for weekend coverage and some hours during the week. The facility specializes in short-term rehabilitation and long-term care.

You will screen patients with cognitive and ADL impairments, develop plans of care, provide therapy, document progress, and participate in discharge planning. Licensed OT with flexibility and commitment to high-quality care is required.

Qualifications

• Licensed Occupational Therapist with current licensure.
• Open to applicants of all experience levels.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop care plans.
• Use standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative treatment per plan and regulatory requirements.
• Administer procedures as part of the rehabilitation plan (ADL retraining, exercises, modalities, devices).
• Assess progress and adjust treatments in clinical records.
• Evaluate outcomes and modify services as needed.
• Provide counseling to patients and families.
• Communicate progress and plans with supervisor and health team.
• Prepare comprehensive discharge summaries.

OCCUPATIONAL THERAPIST - PRN - MISTY WILLOW HEALTHCARE AND REHABILITATION -NORTH HOUSTON, TX

• Weekend Coverage **

Misty Willow Healthcare and Rehabilitation of North Houston is a skilled nursing facility and North Houston's premier location for short-term rehabilitation and long-term care. We are seeking a PRN Occupational Therapist to join our in-house therapy team for weekend coverage and some hours throughout the week.

Key Responsibilities

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient's progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
