Job summary

Elite Home Health is seeking a licensed Occupational Therapist (OT) to provide skilled therapy in patients' homes across Spring Hill and surrounding areas. You will evaluate, plan, implement, and adjust treatments to improve daily living activities and safety, while educating patients and families.

The role requires current Florida OT licensure, CPR certification, and at least one year of OT experience. You’ll collaborate with a multidisciplinary team and supervise OTAs, ensuring Medicare

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, active Florida OT license (OTR) required.
• Minimum of one year of OT experience.
• CPR certification required.
• Valid driver's license and reliable transportation.
• Ability to complete background screening and onboarding.
• Strong clinical judgment and documentation skills.
• Ability to work independently with a collaborative team.

Responsibilities

• Perform initial and ongoing occupational therapy evaluations to assess patients' functional abilities and therapy needs.
• Develop, implement, and update individualized treatment plans in accordance with physician orders.
• Provide skilled therapy interventions to improve daily living skills, functional mobility, safety, and independence.
• Educate patients and caregivers on home safety, adaptive techniques, self-care strategies, and therapeutic activities.
• Assess home environments and recommend appropriate modifications or adaptive equipment.
• Monitor patient progress and adjust treatment interventions as needed.
• Complete accurate and timely clinical documentation, progress notes, reassessments, and discharge summaries.
• Collaborate with physicians, nurses, and other members of the interdisciplinary care team.
• Supervise Occupational Therapy Assistants (OTAs) and Home Health Aides when required by applicable regulations and agency policy.
• Maintain compliance with Medicare requirements, Florida regulations, and agency policies.

What We're Looking For

• Graduate of an accredited Occupational Therapy program.
• Current, active Florida Occupational Therapist (OTR) license required.
• Minimum of one year of occupational therapy experience required.
• Home health, acute care, or rehabilitation experience preferred.
• Current CPR certification required.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to successfully complete required background screening and onboarding requirements.
• Strong clinical judgment, communication, documentation, and organizational skills.
• Ability to work independently while collaborating with a supportive clinical team.
