Bella Vita Health & Rehabilitation in Glendale, AZ is seeking a dedicated Occupational Therapist to join our in‑house therapy team. Responsibilities

• Evaluating patients with cognitive, neurological, orthopedic, and ADL impairments.
• Developing individualized, evidence-based treatment plans.
• Providing skilled occupational therapy interventions including ADL retraining, therapeutic exercise, therapeutic activities, functional mobility training, adaptive equipment instruction.
• Monitoring patient progress and modifying treatment plans to maximize outcomes.
• Collaborating closely with physicians, nursing, families, caregivers, and the interdisciplinary rehab team.
• Educating patients and caregivers for successful transitions home.
• Completing timely and accurate clinical documentation.

What You'll Do
