Job summary

Scylla Solutions is seeking an Occupational Therapist to provide in-home outpatient rehabilitation for older adults. The role emphasizes evaluating patients in their homes, designing and implementing functional therapy plans, and coordinating with families and the rehabilitation team to promote independence.

Responsibilities include supervising staff, adhering to schedules, documenting care, and adapting plans for individual needs to ensure safe, high-quality therapy in diverse home settings.

Qualifications

  • Must hold a valid Occupational Therapist license in the practicing state(s).
  • Must have valid driver’s license or reliable transportation to patient homes.
  • Strong organizational and time management abilities and clear communication skills.

Responsibilities

  • Supervise OT Assistants, temporary licensees, students, and support staff as allowed by state licensure.
  • Maintain adherence to patient treatment schedules and complete clinical and billing documentation.
  • Attend patient care, staffing conferences, and related meetings; contribute to interdisciplinary care planning.
  • Make referrals to other disciplines to address patient needs.
  • Collaborate with rehabilitation team and physicians to select appropriate therapy regimen.
  • Adapt plans of care to patient needs and ensure safety in the home environment.
  • Order supplies and equipment; maintain and report equipment needs and repairs.
  • Focus on patient-centered service and high-quality compassionate care.
  • Screen, examine, and evaluate patients; develop goals, implement plans, and document progress.

POSITION SUMMARY:

The OccupationalTherapist will primarily provide outpatient rehabilitationservices in an in-home settingfor the older adult population. The in-home settingcould be a patient'sprivate residence or in a patient'sroom in an independent living facility. Allows the OccupationalTherapist to evaluate and treat in the most functionalenvironment which is thepatient's home. The OccupationalTherapist organizes and conducts occupationaltherapy programs forthe agency to facilitate rehabilitation. In addition, they direct patient participationin selected tasks torestore, reinforce, and enhance performance. The OccupationalTherapist facilitates the learning ofthose skills and functionsessentialfor adaptationand productivity, in order to diminish or correctpathology. This positionprovides a flexible schedule and work environment in various settings.

RESPONSIBILITIES/ACCOUNTABILITIES:

As necessary or applicable, supervision of Occupational Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professionalstandards.

Responsible for adherence to department and patient treatment schedule, timely clinical and billing documentation requirements as well as work related meetings.

Attends and contributes to patient care, staffing conferences and other related meetings.

Makes referrals to other disciplines, agencies in order to address the comprehensive needs of the patient.

Consults with other members of the rehabilitationteam, including the community physician, toselect the most appropriate therapeuticregimen consistent with the needs and capabilitiesof eachpent.

Adapts program and/or plan of care according to the needs of individual patients.

Promotes a safe environment and enforces the eliminationof fire and safety hazards.

Orders supplies and equipment as necessary.

Cleans, maintains, identifies, and reports the need for repair of equipment as necessary.

Puts patient service first: ensures that patients and families receive the highest quality of service ina caring and compassionate atmosphere which recognizes the individuals' needs and rights.

Screens, examines and evaluates patients, including history, systems review, and applicationofappropriate tests and measures, synthesis informationand interprets examinationfindings in orderto establish a diagnosis, identifyimpairments, determine the predicted level of improvement andtheme required to achieve it, identifyprecautions/contraindicationsand design and plan of care.Develops appropriate treatment goals and methods in collaborationwith the patient andcaregivers, implements the occupationaltherapy treatment plan, and completes all related documentationand record keeping regarding these services. Services include but are not limited to applicationof therapeutictechniques in areas such as:

Designs, fabricates, fits and/or applies adaptivedevices, and trains patients in the use of adaptivedevices including orthotics and prosthetics.

Selects appropriate activitiessuited to the individual's physical and mental capacity, intelligencelevel and interest.

Instructs patients in the skills and techniques required for participationin functionalactivitiesandevaluates patient progress, attitudes, and behavior as related to rehabilitativepotential.

Performs routinereexaminationas needed/required to modify/progress plan of treatment.

Performs other related duties as required.

SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS:

1. Must have a valid OccupationalTherapist license in the state(s) of practice, or proof of license eligibility.

2. Must have a valid driver's license or reliable, independent transportationto and from the patients'residence.
