Personal Touch Home Care in Fort Thomas, KY is seeking compassionate Occupational Therapists to assess patients, develop and implement treatment plans, and help clients improve daily living and work skills in the comfort of their own homes.

The role focuses on promoting mobility, function, and independence with rehabilitative services delivered in patients' homes. A valid OT license, accredited degree, and preferred long-term/geriatric care experience are required for this full-time position.

Qualifications

• Must have graduated from an accredited Occupational Therapy college or university.
• Must hold a valid OT license and state registration.

Responsabilités

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities, and rehabilitation potential.
• Establish and administer a treatment program under physician direction.
• Set realistic and achievable goals for patients.
• Document and record patient condition.
• Educate patients and families in appropriate therapy methods.
• Complete evaluations, weekly progress notes, re-certification, and discharge documentation.
• Collaborate with all disciplines to provide patient-centered care.
• Perform duties as assigned by the Director of Rehabilitation.
• Comply with confidentiality, HIPAA, and Residents' rights.

Job Overview:

As an Occupational Therapist with Personal Touch Home Care, you will be responsible for assessing patients' needs, developing and implementing treatment plans, and helping clients improve their daily living and work skills. You will also provide rehabilitative services to patients who are unable to travel to a clinic or hospital. This is focused on improving patients' quality of life by promoting mobility, function, and independence within the comfort and convenience of their own homes. Your responsibilities will include:

• Responsible for the assessment of referred patients that require rehabilitation services


• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation


• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician


• Set realistic and achievable goals for their patients


• Document and record the patients' condition


• Educate patients and families in an appropriate therapy method.


• Evaluations, weekly progress notes, re-certification and discharge documentations


• Collaborates with all disciplines to provide best patient centered treatment and care


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights

Qualifications:

• Must have graduated from an accredited Occupational Therapist college or university


• Must have a valid professional license and registration for state of practice


• Experience with long-term care and/or geriatric care is preferred but not limited to


• Demonstrates good verbal and written communication and organization skills.


• Prefer two (2) years of appropriate experience as an occupational therapist. Home health experience is preferred.

Job Types: Full-time

Pay: $68.00 through $85.00
