Job summary

Storybrook Care & Rehabilitation in Northern Colorado, Fort Collins area, is seeking a compassionate Occupational Therapist to join our in-house therapy team at a midsize long-term care facility. The role supports cognitive and ADL functioning needs while offering a family-friendly environment and a varied schedule.

The OT will screen and evaluate patients, develop and adjust plans of care, use standardized assessments, and document outcomes.

Responsibilities

  • The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements.
  • This includes recovery, improvement in function and when possible, a return to a prior level of functioning.
  • Clinical documentation will describe objective measures and justification for continued or discontinued services.
  • Effectively screens/evaluates patients and/ with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
  • Utilizes standardized assessments and evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
  • Assesss patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
  • Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
  • Participation in required meetings including but not limited to: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
  • Participate in facility and company required trainings, in-services and conferences.
  • Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
  • Adherence to NBCOT /AOTA and State Required practice acts, code of conduct, ethics during professional practice as an Occupational Therapist.
  • Adherence to Company required policy and procedures and code of conduct.
  • Active State Licensure

Our family owned and operated long term care facilitator is seeking a compassionate and well versed an Occupational Therapist for consideration to add to the inhouse therapy team a midsize facility situated in Northern Colorado, amongst Larimer County. This building is home to a diverse group of long-term residents, cared for by seasoned staff with low staffing ratios, who are stewarded by a well-equipped clinical team and Administrator with years of experience in the geriatric field.

Principal Responsibilities
