CommonSpirit Health at Home is seeking a skilled Occupational Therapist to join the Deaconess Home Health team in Evansville, Indiana. This part-time role provides home-based therapy with a focus on improving independence and quality of life for patients.

You will assess, plan, and implement individualized treatment, educate patients and families on safety and adaptive equipment, and maintain compliant records.

Competenze

• Bachelor's degree from an accredited school approved by the American Occupational Therapy Association (AOTA).
• Active registered OT license in the state where you practice.
• Current CPR/BLS certification.
• One year of Occupational Therapy experience in a clinical setting.
• Current Driver's License.
• Comply with federal, state and local laws and regulations.

• Teach patients and families about home safety, adaptive equipment, and daily activity strategies.

At Deaconess Home Health, we strive to embody our mission of delivering hope and healing to those we serve. We are seeking a skilled and compassionate Occupational Therapist to join Deaconess Home Health team. The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises’, adaptive techniques, and use of assistive devices to improve patients functional abilities.
• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
• Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
• Monitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.
• Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.

Job Requirements

• Bachelor's degree from an accredited school approved by the American Occupational Therapy Association
• Active registered OT license in the state which you practice
• Must obtain and maintain current CPR/BLS Certification
• One-year Occupational Therapy experience in a clinical setting.
• Current Drivers License
• Comply with federal, state and local laws and regulations
