Job summary

CommonSpirit Health at Home is seeking a skilled Occupational Therapist (PRN) to deliver home-based therapy, assess needs, and help patients regain independence and quality of life.

Responsibilities include evaluating patients, developing treatment plans, teaching home safety, and coordinating with the care team to ensure optimal outcomes.

Qualifications

• Bachelor's degree from an accredited OT program.
• Active OT license in the practicing state.
• Current CPR/BLS certification.
• One year OT experience in a clinical setting.
• Current driver's license.
• Comply with federal, state and local laws and regulations.

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs and develop personalized treatment plans.
• Teach patients and family members about home safety, adaptive equipment, and strategies for daily activities.
• Maintain accurate and timely records of patient progress, treatment plans, and plan modifications for compliance and regulatory standards.
• Monitor patient progress and adjust treatment plans as needed, using outcome-based measures to assess effectiveness.
• Collaborate and communicate patient changes in condition with physicians, nurses, and other home health team members.

We are seeking a skilled and compassionate* Occupational Therapist - PRN *to join Deaconess Home Health team. The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises’, adaptive techniques, and use of assistive devices to improve patients functional abilities.
• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
• Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
• Monitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.
• Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.

Job Requirements

• Bachelor's degree from an accredited school approved by the American Occupational Therapy Association
• Active registered OT license in the state which you practice
• Must obtain and maintain current CPR/BLS Certification
• One-year Occupational Therapy experience in a clinical setting.
• Current Drivers License
• Comply with federal, state and local laws and regulations
