You will conduct assessments, implement interventions, and collaborate with a multidisciplinary team to ensure coordinated care for residents in a nursing/rehabilitation setting.

Qualifications

• State licensure or ability to obtain licensure.
• Strong interpersonal and communication skills.
• Ability to work independently and within a multidisciplinary team.

Responsibilities

• Conduct comprehensive assessments and develop individualized treatment plans.
• Implement therapeutic interventions to enhance clients' functional abilities.

• Educate clients and caregivers on home exercises and adaptive techniques.
