Job summary

Live Well Homecare is seeking an experienced occupational therapist to provide patient-centered OT services across home and community settings.

You will assess functional needs, collaborate with a multidisciplinary team, educate patients and families, and participate in quality improvement efforts. A current state license or eligibility is required, along with a strong focus on daily activity performance and clear communication.

Qualifications

• Bachelor's degree or higher from an accredited occupational therapy program.
• Licensed or eligible for licensure to practice in the relevant state.
• Strong understanding of physical, cognitive, psychosocial, and sensory factors impacting daily activities.
• Excellent communication and patient education skills.
• Ability to work effectively within a team and adapt to various clinical settings.
