Job summary

Mentor South Bay is seeking an Early Intervention Occupational Therapist to coordinate treatment and implement services for assigned clients as part of a multidisciplinary Early Childhood Services team.

The role involves outreach visits, collaboration with team members to develop individualized plans, maintaining comprehensive records, and ensuring billing and documentation compliance across both part-time and full-time schedules.

Qualifications

  • Current licensure as an Occupational Therapist by the Massachusetts Board of Registration of Allied Health Professionals.

Responsibilities

  • Provide outreach services to children and families
  • Coordinate treatment and implement services with the Early Childhood Services team
  • Maintain client records including IFSPs with assessments and goals
  • Document client progress and contact with collaterals and families
  • Complete billing documentation per company schedule
  • Adhere to professional standards and guidelines for each discipline
  • Keep supervisor informed of progress on all clients
  • Conduct visits with clients in natural environments as needed

Early Intervention Occupational Therapist

Responsible for coordinating treatment and implementing services for assigned clients (including outreach visits) as a member of a multi-disciplinary Early Childhood Services team.

Primary Functions
  • Provide outreach services to children and families
  • Consult with team members in the development of individualized plans and multidisciplinary services for assigned clients
  • Maintain client records completely and accurately to include IFSP’s with full assessment of client functioning and goals
  • Documentation of client progress in the program, records of required contact with collaterals and family members
  • Documentation of arena assessments and documentation of DPH forms and regulations regarding intake and discharge of clients
  • Complete billing documentation according to schedule established by the company
  • Adhere to professional standards and guidelines according to their respective disciplines
  • Keep supervisor informed of progress of all clients
  • Meet direct service requirements based on part time or full time status, as designated
  • Adhere to clinical recommendations from clinical administration
  • Maintain confidentiality in all clinical matters
  • Remain compliant with billing and documentation in accordance with direct service requirement
  • Conduct visits with clients and relevant collaterals in natural environments according to the needs of the child and family
Qualifications
  • Current licensure as an Occupational Therapist by the Massachusetts Board of Registration of Allied Health Professionals.
