Job summary

German Centre is seeking an Occupational Therapist to evaluate residents and develop individualized treatment plans to improve independence and quality of life. The role involves providing skilled OT interventions to enhance ADLs, mobility, balance, and safety, while collaborating with physicians, nursing staff, and families to set goals and deliver coordinated, resident-centered care.

• Graduate of an accredited occupational therapy program.
• Current, active MA Occupational Therapist license
• Minimum 1 year experience, preferably in a long-term care setting
• Ability to communicate fluently in English – both verbally and written

Responsibilities

• Evaluate residents and develop individualized treatment plans to improve independence, functional abilities, and quality of life.
• Provide skilled occupational therapy interventions to enhance ADLs, upper extremity function, mobility, balance, and safety.
• Collaborate with physicians, nursing staff, rehabilitation team members, and families to establish goals and deliver coordinated, resident-centered care.
• Recommend and train residents and caregivers in the use of adaptive equipment, environmental modifications, and fall prevention strategies to promote safe, independent living.
• Complete timely, accurate documentation, monitor resident progress, adjust treatment plans as needed, and maintain compliance with regulatory and facility standards.
