Job summary

LiveWell Homecare Agency, LLC is seeking an Occupational Therapist to provide rehabilitation services for individuals with brain, spinal cord, and complex injuries in community-based settings. You will evaluate clients, develop treatment plans, and collaborate with families and physicians to optimize outcomes.

The role supports full-time, part-time, and flexible schedules, with a comprehensive benefits package including 401(k), paid time off, and holiday pay, plus sign-on bonus and licensure

Qualifications

• Bachelor’s Degree in Occupational Therapy is required with one year of human services experience.
• Licensed OT/L in the state and member of AOTA.
• Valid driver’s license, vehicle registration, and auto insurance.

• Re-evaluate progress, adjust programs, and manage caseloads.
• Create home exercise programs and conduct home evaluations.

• Collaborate with physicians, families, and community resources to ensure successful therapy outcomes.
