Job description

Facility in Layton, UT seeking ObGyn locums coverage Rates: $2500/day with 4 hour gratis, $350/hr after 4 hours Details:

• Coverage Start Date: As soon as credentialed
• Setting: Outpatient, Inpatient, ER, Clinic, Call
• Reason for Coverage: Backfill needed due to the loss of two OB/GYN providers
• EMR: EPIC
• Support Staff: Medical Assistant (MA)
• Required Procedures: Gyn and OB patients, including surgeries and assisting in surgery
• Trauma Level: N/A
• Beds: 220 beds

Schedule/Availability:

• Clinic: Monday, Tuesday, Thursday from 8 AM to 5 PM
• Call: 24-hour call shifts (7 AM-7 AM), includes Wednesdays and 2 weekends per month (Friday-Monday)
• Call Back: 0-2 times per shift
• Shifts/Month: 7-10 call shifts per month, clinic schedule is flexible

Requirements:

• Board Certified OB/GYN (ABOG): Required
• Active UT License: Required (IMLC with LOQ acceptable)
• BLS: Required (or willingness to obtain)
• DEA: Required
• Shifts Per Month: Must state the number of shifts the candidate can offer
• Clean Malpractice: Required (up to $100k acceptable with explanation)
• NPDB Self-Inquiry: Required
• Clean Background: Required
• EMR Training: Must complete required training modules before starting

Credentialing Timeline: 90 days after receiving a completed application
