Join us in building a mission-driven women's health department serving California's Central Valley.

Living Water Clinic is a growing, multi-site healthcare organization serving Tulare and Fresno counties. Through our Rural Health Clinics, we provide primary care, dental, behavioral health, and women's health services to a largely bilingual and historically underserved patient population in agricultural communities. We are also expanding our private practice and PPO-participating clinic footprint.

We are a faith-based practice grounded in Christian values, seeking board-certified or board-eligible OB/GYN physicians who want to help build something meaningful.

THE OPPORTUNITY

We are actively expanding our women's health team and have the capacity to bring on two to four full-time OB/GYN physicians over the next 18 to 24 months, alongside certified nurse-midwife (CNM) support and future physician assistant integration.

The immediate opportunity includes succeeding our current OB/GYN, who is transitioning to a reduced practice schedule. This offers the incoming physician an established women's health patient panel, existing hospital relationships, and a functioning clinical workflow. Hospital privileges would be subject to the incoming physician's credentialing and approval.

Our vision also includes an ACGME-accredited family medicine residency program, with the first class targeted for fall 2027 and women's health as a core rotation. This would create meaningful teaching opportunities for physicians interested in academic engagement.

Our practice combines Rural Health Clinic sites serving underserved populations with private and PPO-participating clinics, providing additional growth opportunities for physicians interested in a broader payer mix and a range of clinical complexity.

COMPENSATION
  • Base compensation: $500,000 to $650,000+ annually, potentially reaching $700,000+ for candidates with subspecialty training, an interest in leadership roles, or established local patient panels. Compensation depends on experience, credentials, scope of practice, hospital-based responsibilities, and call participation.
  • Structure options: 1099 independent contractor engagement through a professional corporation or W-2 employment, based on candidate preference and practice fit.
  • Additional incentives: Performance and productivity bonuses, a sign-on bonus, relocation assistance, and physician referral bonuses.
  • Malpractice coverage: Tail coverage options are subject to negotiation.
  • Continuing education: CME allowance and paid time.
  • Retirement: 401(k) participation with an employer match and cash balance plan eligibility.
HPSA DESIGNATION, LOAN REPAYMENT, GRANTS, AND SCHOLARSHIPS

All Living Water Clinic sites are located within federally designated Health Professional Shortage Areas (HPSAs). Physicians may qualify for the following programs, subject to each program's requirements, including applicable site, employer, and service eligibility criteria:

  • National Health Service Corps (NHSC) Loan Repayment Program: Up to $50,000 for a two-year, full-time service commitment, with opportunities for additional years of participation.
  • NHSC Students to Service Loan Repayment Program: For medical students committing to service after training.
  • Steven M. Thompson Physician Corps Loan Repayment Program (California): Up to $105,000 for a three-year service commitment in a California HPSA.
VISA SPONSORSHIP

Living Water Clinic is willing to sponsor qualifying international physicians, including J-1 visa waiver placements through California's Conrad 30 program.

Spanish fluency is highly valued given our patient population, but it is not required for consideration.

PRACTICE STRUCTURE AND LIFESTYLE

We are building this department to support a sustainable, long-term practice, with intentional attention to workflow, coverage, and physician wellness:

  • Call schedule: The structure will depend on the size of the OB/GYN team at the time of hire and will incorporate collaboration with other community OB/GYN groups where appropriate. Our objective is to build a sustainable coverage model that provides meaningful physician recovery time and avoids continuous solo call coverage.
  • CNM and team support: Certified nurse-midwife support for labor management and low-risk deliveries, with outpatient support from CNMs, future physician assistants, and, eventually, family medicine residents.
  • Post-call protection: When call responsibilities include overnight or extended shifts, we build in protected recovery time to support physician wellness and patient safety.
  • Scheduling flexibility: A standard four- to five-day workweek is negotiable. Part-time and hybrid arrangements will be considered for experienced physicians.
  • Hospital affiliations: Sierra View Medical Center in Porterville and Kaweah Health Medical Center in Visalia offer access to labor and delivery services and operating rooms within driving distance of clinic sites, subject to individual credentialing and privileges.
  • Team-based care: Collaboration with primary care, behavioral health, dental, and pharmacy teams within an integrated clinical system.
CLINICAL SCOPE

The role's scope is flexible, depending on candidate interests and practice fit:

  • Full-scope OB/GYN: Prenatal care, labor and delivery, cesarean sections, gynecologic surgery, and comprehensive outpatient women's health care.
  • Outpatient-focused OB/GYN: Prenatal care, well-woman care, gynecologic evaluations, and minor procedures, with hospital-based services shared or referred.
  • Subspecialty and focused clinical interests: Interests in maternal-fetal medicine, minimally invasive gynecologic surgery, colposcopy, urogynecology, and related areas can be incorporated into the role.
LOCATION AND COMMUNITY

Our clinic sites include Lindsay, Woodlake, Selma, and Porterville, with expansion planned for additional California communities, including Corning.

The Central Valley offers a cost of living materially below that of coastal California, with access to Sequoia National Park, the Sierra Nevada, and the amenities of the greater Fresno metropolitan area.

Approximately 60% of our patients are Spanish-speaking, making Spanish fluency particularly valuable.

WHAT YOU BRING
  • Board certification in obstetrics and gynecology, or board eligibility with a clear pathway to certification.
  • An active California medical license or eligibility for California licensure.
  • DEA registration.
  • An interest in serving underserved, rural, and bilingual patient populations.
  • Spanish fluency, preferred but not required.
