Job summary

Memorial Health System in Marietta, OH, seeks a Women’s Health Patient Navigator to coordinate non-clinical patient support and clinic workflows throughout the OB/GYN service line. Based in the clinic, the navigator helps patients arrange appointments, referrals, and follow-ups from prenatal care through postpartum and gynecologic services.

The role maintains registries, tracks screenings, and escalates urgent issues to the team while safeguarding privacy.

Qualifications

• Bachelor’s degree as specified.
• Minimum of 3 years healthcare experience in ambulatory operations, patient navigation, scheduling, or care coordination.
• Experience in OB/GYN, maternal health, preventive services, or quality improvement preferred.

Responsibilities

• Coordinates non-clinical navigation across OB/GYN clinic locations, serving as a point of contact for patients and supporting timely appointments, referrals, records, and follow-up under clinic workflows.
• Tracks PRAF workflows and prompts for obstetrical provider to complete assessment, monitors submission, resolves gaps without clinical assessment.
• Monitors follow-up lists for maternal and preventive care, routes clinical questions to licensed staff.
• Maintains patient registries, work queues, and tracking tools for prenatal and postpartum care and related documentation.
• Contacts patients to arrange visits and connect them with transportation, financial, and community resources; documents outreach and escalates urgent concerns.
• Works with providers and staff to close administrative handoff gaps during prenatal, gynecologic, surgical, and postpartum care.
• Tracks sterilization and consent documents and flags missing items before scheduling.
• Supports coordination for women’s health programs and partnerships with other hospitals.
• Coordinates placenta donation referrals including consent documentation.
