Job summary

Mobridge Regional Hospital seeks an OB Nurse Navigator (RN) to support patients through pregnancy and postpartum by coordinating care, educating patients, and helping with referrals. Qualifications

• Current South Dakota RN license and CPR certification.

• BSN and/or maternal-child health certification preferred.

Job description

The OB Nurse Navigator is an RN who supports patients throughout pregnancy and the postpartum period by coordinating care, providing education, assisting with referrals, and helping patients navigate the healthcare system. This position works closely with OB providers, clinic and hospital staff, specialists, and community resources to promote positive outcomes for mothers and babies.

Key Responsibilities

• Serve as a primary point of contact and advocate for OB patients throughout pregnancy and postpartum.
• Coordinate prenatal, diagnostic, specialty and postpartum appointments, testing and referrals.
• Help patients understand their care plan, appointments, testing and follow-up needs.
• Identify barriers to care, including transportation, financial concerns, insurance, childcare and other social needs.
• Coordinate referrals to specialists, maternal-fetal medicine and community resources.
• Follow up on referrals, missed appointments and recommended care.
• Facilitate communication between patients, providers, hospital departments and community resources.
• Provide education on prenatal care, childbirth, postpartum recovery, newborn care and breastfeeding/lactation.
• Educate patients about pregnancy and postpartum warning signs and when to seek medical attention.
• Provide support and connect patients with social services, behavioral health and other resources as needed.
• Assist with care coordination for high-risk pregnancies and patients with complex needs.
• Assist with delivery preparation and postpartum follow-up.
• Maintain accurate and timely documentation in the electronic medical record.
• Participate in quality improvement activities and track program outcomes.
• Maintain professional communication and relationships with patients, families, providers and staff.
• Participate in continuing education related to obstetrics, maternal-child health and care coordination.

Qualifications

• Current South Dakota Registered Nurse license.
• Current CPR certification, or obtain certification as soon as offered through MRH&C.
• Strong communication, organizational and interpersonal skills.
• Previous experience in obstetrics, women's health, prenatal/postpartum care or care coordination preferred.
• BSN and/or maternal-child health certification preferred.

Physical Requirements

Ability to stand, walk, bend and stoop as needed; assist with patient movement when necessary; and use a computer and electronic medical record for extended periods.
